Senior Adults & Persons with Disabilities
Long-Term Care Consultations are provided to persons with disabilities and aging adults to guide placement decisions. Home and Community Based Waiver programs help eligible persons live safely in their homes with supports. Case management, adult protection, information and referral services are also provided.
