Birth and Death Records


You may request a copy of a birth or death certificate:
  • By visiting our office
  • Request a copy by mail
  • Download and complete an application
  • Mail or bring it to the Recorder’s Office in the Cook County Courthouse with the appropriate fee

Birth Certificates
Birth records for individuals born in Minnesota on or after 1935 are available in any county within the State of Minnesota. Use the Birth Certificate Application to order a certificate. Print a Copy and complete the application, then mail or bring a copy along with the appropriate fees to the Recorder's Office located in the Cook County Courthouse.
Death Certificates
State death records beginning in 1908 are available in any county within the State of Minnesota. If you want a copy of a death record prior to those years please contact the Recorder’s Office in the County where the death occurred. Use the Death Certificate Application to order a certificate. Print a copy and complete the application, then mail or bring in a copy along with the appropriate fees to the Recorder's Office Located in the Cook County Courthouse.

Out-of-State Birth Certificates
If you were not born in Minnesota you can contact the National Center for Health Statistics for more information about ordering an out-of-state birth certificate.
